How can something be safe and effective and kill people?
In short it can't, you can dress it up anyway you like, but if a medication kills one person it cannot be deemed safe nor effective. Killing one person to save another is not justifiable nor quantifiable. You can never know how many it saved as they were alive before taking the medication so there is zero evidence the medication saved them. However those that died after medication there is a lot of evidence not least the fact they were alive before taking the medciation and died after.
Do not quote something as safe if one of the reactions to it is death. It is then a risk. Being brainwashed into thinking it's safe because they say so, doesn't render it safe. They redefined what Safe and effective really means. It now means with risks of heart attacks, blood clots.
